1. How quickly will you respond?

Like most NDIS community service providers, Relay Your Care typically acknowledges and responds to new referral inquiries within 24 to 48 business hours to initiate the intake screening process.

Locations: They currently have active capacity to support NDIS participants across Brisbane, Moreton Bay, Caboolture, Morayfield, North Lakes, Redcliffe, and surrounding areas.

 

Services: They are accepting referrals for:

    • Assistance with Daily Living / Daily Personal Activities

    • Community Access & Social Support / Group and Centre-Based Activities

    • Household Tasks (Cleaning, meal prep, maintenance)

    • Life Skills Development

    • Travel & Transport Assistance

    • Support Coordination

    • Access and Maintain Employment / Higher Education Support

Yes. You can initiate a referral inquiry to check for service compatibility and slot availability. However, service delivery and the signing of the formal Service Agreement cannot fully commence until NDIS funding is confirmed and allocated to the relevant support categories.

To expedite the onboarding process, you should attach:

    • The participant’s current NDIS Plan (specifically the goals and funding sections).

    • Any relevant Allied Health reports, occupational therapy assessments, or behavior support plans.

    • Risk assessments or medical management plans if applicable.

 

Yes, absolutely. Relay Your Care accepts direct self-referrals from participants, as well as referrals made on their behalf by family members, nominees, Support Coordinators, or Allied Health professionals.

Updates regarding the status of the referral are communicated directly via email or phone call to the primary contact person listed on the referral form (whether that is the participant, coordinator, or guardian).

If they do not have the capacity, do not service the specific geographical area, or cannot safely meet the participant’s complex needs, they will formally notify the referrer. They will usually provide information on alternative avenues or let you know when their capacity limits are expected to change.
