What is Community Participation?

Community Participation is a tailored support service aimed at empowering individuals to actively engage in social, recreational, and civic activities. The goal of this program is to break down barriers to participation, reduce social isolation, and promote a genuine sense of belonging within your community.

Through our experienced and compassionate support workers, clients receive the customized assistance they need to explore their hobbies, attend appointments, and build skills that enhance their independence in public and social settings.

Who Can Access This Program?

This program is available to NDIS participants with approved funding for relevant supports, as well as older Australians who have been assessed through My Aged Care and are eligible for Support at Home services.

What is Community Access support under the NDIS?

Community Access support helps NDIS participants get out into the community, take part in activities, build social connections and develop greater confidence and independence. Support is tailored to each person’s goals, interests and individual needs.

What activities can Community Access support include?

Depending on your goals and preferences, Community Access support may include shopping, appointments, social outings, recreational activities, community events, group activities and other activities that are important to you.

Can I choose the activities I participate in?

Yes. You have choice and control over the activities you take part in. We work with you to understand your interests, preferences and goals rather than simply choosing activities on your behalf.

Can Relay Your Care provide transport for Community Access?

Transport may be provided as part of your Community Access support where appropriate and agreed in advance. We will explain any applicable transport arrangements or charges before your services commence.

Can I receive one-to-one Community Access support?

Yes. Community Access support can be provided one-to-one and personalised around your individual needs, goals, preferences and available NDIS funding.
