What is Community Participation?
Community Participation is a tailored support service aimed at empowering individuals to actively engage in social, recreational, and civic activities. The goal of this program is to break down barriers to participation, reduce social isolation, and promote a genuine sense of belonging within your community.
Through our experienced and compassionate support workers, clients receive the customized assistance they need to explore their hobbies, attend appointments, and build skills that enhance their independence in public and social settings.

Who Can Access This Program?
This program is accessible to individuals who are participants of the National Disability Insurance Scheme (NDIS) with approved funding for Core Supports (Assistance with Social and Community Participation) or Capacity Building, as well as older Australians who have been assessed through My Aged Care and are eligible for social support services.
We tailor our involvement to meet the unique goals, needs, and funding arrangements of each individual.

What is the Community Participation program?
Community Participation is a tailored service designed to help you confidently engage in social, recreational, and civic activities. While having an active NDIS plan is the most common way our participants access disability support for this program, it is not always strictly required. We also accommodate self-funded clients and can provide services regardless of your NDIS involvement. If you are in the process of applying for the NDIS, we are happy to point you in the right direction.

This program is ideal for NDIS participants who have funding allocated for Assistance with Social and Community Participation (Core Supports or Capacity Building), as well as older Australians accessing My Aged Care. We also warmly welcome self-funded individuals who are looking for extra support to stay active, social, and connected to their local community.
How do I apply?
We aim to get you the support you need as smoothly and quickly as possible. Once you submit an enquiry and we complete your initial care consultation, support can often begin within a few days to a week. The exact timeline will depend on the complexity of your care plan and matching you with the perfect support worker.
Can I change providers?
Absolutely. We believe that a strong, comfortable relationship is the foundation of good care. We will carefully match you with support workers based on your specific needs, personality, and preferences. You will always have the opportunity to meet and approve your support worker before your services officially commence to ensure they are the right fit for you.
What services can Relay Your Care provide?
For our aged-care clients, we work in alignment with Trilogy Care, a registered Home Care Package (HCP) provider that specializes in self-managed and coordinated home care.
Trilogy Care handles the administrative side—such as care planning, clinical support, and budget management—for a low, flat percentage fee with no hidden costs. This setup maximizes the funds you have available for actual services. It gives you the freedom and flexibility to choose your own local support workers and schedule the exact care you want, keeping you in total control of your aged-care journey.
