What is Transport & Social Support?

Transport and Social Support is a dedicated service aimed at helping individuals navigate their community safely and confidently. The goal of this program is to ensure that a lack of mobility or access to a vehicle never stands in the way of your essential needs or social life.

Our experienced support workers provide comfortable door-to-door transport and can also accompany you during your outings to provide physical assistance, guidance, or simply good company. We can also assist clients getting to and from the vehicle and accessing community activities according to their individual support needs.

Who Can Access This Program?

This program is available to NDIS participants with appropriate funding for Transport or Assistance with Social and Community Participation. Eligible older Australians may also access transport and social/community support through Support at Home where these services are included in their approved support arrangements.

 

We tailor our support to meet the unique travel needs, goals, and funding arrangements of each individual. We also accept self-funded clients and can discuss support options based on your individual circumstances.

Yes. Where transport forms part of your agreed supports, Relay Your Care can assist you to travel safely to appointments, activities, shopping, social outings and other community destinations that align with your needs and goals.
Transport support may include travel to medical or allied health appointments, shopping, social activities, community programs and other agreed destinations connected to your individual support needs and goals.
Yes, where this is included in your agreed support. Your support worker can assist you before, during and after an appointment or community activity, depending on your individual needs.
Depending on your service arrangement and funding, additional transport-related costs may apply. We will discuss any applicable charges with you and agree on them before your services commence.
Yes. Transport Assistance can form part of your Community Access support, allowing your support worker to assist you throughout the activity rather than simply providing transportation to and from a destination.
