What is the Support at Home Program?
The Support at Home Program is an Australian Government support at home program designed to help older Australians remain living safely and independently in their own homes.
The program provides funding for approved services based on each person’s assessed needs, goals, and preferences.
Through our partnership with Trilogy Care, clients can access funding management, care planning, and coordinated support while choosing the services that best suit their lifestyle.

What is the Support at Home Program?
While having an active NDIS plan is the most common way our participants access disability support, it is not always strictly required. We also accommodate self-funded clients and can provide services regardless of your NDIS involvement. If you are in the process of applying for the NDIS, we are happy to point you in the right direction.
Who is eligible?
We currently provide dedicated community and home care services across Sydney and Brisbane. If you are located on the outskirts of these areas, please get in touch with our team to discuss potential support arrangements.
How do I apply?
We aim to get you the support you need as smoothly and quickly as possible. Once you submit an enquiry and we complete your initial care consultation, support can often begin within a few days to a week. The exact timeline will depend on the complexity of your care plan and matching you with the perfect support worker.
Can I change providers?
Absolutely. We believe that a strong, comfortable relationship is the foundation of good care. We will carefully match you with support workers based on your specific needs, personality, and preferences. You will always have the opportunity to meet and approve your support worker before your services officially commence to ensure they are the right fit for you.
What services can Relay Your Care provide?
For our aged-care clients, we work in alignment with Trilogy Care, a registered Home Care Package (HCP) provider that specializes in self-managed and coordinated home care.
Trilogy Care handles the administrative side—such as care planning, clinical support, and budget management—for a low, flat percentage fee with no hidden costs. This setup maximizes the funds you have available for actual services. It gives you the freedom and flexibility to choose your own local support workers and schedule the exact care you want, keeping you in total control of your aged-care journey.
