What is the Support at Home Program?

The Support at Home Program is an Australian Government program designed to help older Australians remain living safely and independently in their own homes.

The program provides funding for approved services based on each person’s assessed needs, goals, and preferences.

Through our partnership with Trilogy Care, clients can access support with funding, administration and care-planning arrangements while choosing services that best suit their individual needs, goals and preferences.

Who Can Access This Program?

Eligibility for government-funded Support at Home is determined through an aged care assessment accessed through My Aged Care. The assessment considers each person’s individual needs and circumstances to determine the appropriate support available to them.

Yes. Support at Home is designed to help eligible older Australians remain safe, comfortable and as independent as possible in their own homes. We provide personalised in-home support based on your needs, preferences and agreed support arrangements.
Yes. Depending on your assessed needs and support arrangements, we can assist with personal care activities such as showering, dressing, grooming, toileting and other agreed personal activities.
Yes. Our domestic assistance services can include reasonable household tasks such as cleaning, laundry and other agreed activities that support your wellbeing and ability to live independently at home.
Yes. Depending on your approved support arrangements, services may include shopping, transport, appointments, social activities and community participation. We can discuss your individual needs and the supports available to you.
Government-funded Support at Home eligibility is determined through an aged care assessment accessed through My Aged Care. Once the appropriate arrangements are in place, services are coordinated according to your assessed needs, preferences, available funding and arrangements with your approved provider or care partner.
